Why It Happens: Renewal notice sent to old address goes unanswered.
How to Fix It: Update your address at azahcccs.gov or call 1-800-654-8713
Missed Renewal Deadline
Why It Happens: Renewal paperwork not returned in time
How to Fix It: Reapply immediately at healthearizonaplus.gov – coverage can often be reinstated if caught quickly
Income Not Reported or Changed
Why It Happens: A job change or raise wasn’t reported, triggering a review
How to Fix It: Report updated income through Health-e-Arizona Plus – you may still qualify at the new income level
Missing or Incomplete Documents
Why It Happens: Required documents (ID, proof of residency) weren’t submitted with renewal

2. Gather documents (ID, proof of income, proof of residency)
3. Complete the renewal application:
